Look closer at the medicine

# How does a made-to-order medicine differ?

“Compounded” means made to order. Ask why a preparation made for you is needed and who is responsible for preparing it.

## Why might compounding come up?

Compounding can serve a patient whose needs an approved medicine cannot meet, such as someone allergic to a dye in the finished product.

Ask how the proposed change meets your own medical need.

- What part of the approved option does not meet my need?

- What exactly would be changed?

- Who is making that decision and recording the reason?

## What has not been checked?

The Food and Drug Administration, or FDA, has not approved the medicines made to order.

The FDA does not assess how well these medicines work or check their quality and safety before sale.

- Ask for the name of the business that actually prepares the medicine.

- Ask who can explain its label and written instructions.

- Ask how questions or problems will reach the prescriber and pharmacist.

## Why do copied weight medicines need more questions?

During a national shortage, pharmacies may be permitted to copy a medicine whose copying would otherwise be restricted.

Semaglutide injections and tirzepatide injections were both off the FDA’s national shortage list by September 26, 2026. Pharmacies could no longer rely on those shortages as permission to make copies.

For ordinary compounding pharmacies, the law limits regular or large-scale copying of a medicine already sold as a ready-made product.

The person prescribing can decide that a change is important for one named patient. The pharmacy must still use ingredients allowed for compounding and have a certificate showing their tested quality.

Removing a dye because of a patient’s allergy is an example of a change that may meet a medical need.

Simply costing less does not qualify as this type of patient-specific change.

- Ask what makes the proposed change matter to your particular medical need.

- Ask the prescriber and pharmacist why the proposed medicine meets the rules for making it to order.

- If the answer rests on a shortage, ask to see the FDA’s current national shortage list.

## Bring the question back to you

What need of mine would this made-to-order medicine meet that an approved medicine cannot?

### Keep this in mind

Ask what calls for medicine prepared specifically for you, then ask which pharmacy would prepare it. Write down both answers.

US care facts as of September 2026.
